This module is part of a series value-based payments for behavioral health providers. The module reviews the concept of care pathways and approaches for creating and implementing care pathways for various clinical health and social determinant needs. After taking this course, participants should be able to: recognize the role of care pathways in providing the effective and efficient care needed for successful adoption of value-based payments, identify the components of a care pathway, specify how the design of a care pathway supports a clinical and administrative work flow efficiency and effectiveness, identify the steps for implementing a care pathway, identify common challenges and solutions to care pathway implementation.
